Freelance change request form

When a client asks for "one more thing," send them this intake form instead of saying yes on Slack. They fill title, description, priority and desired date in the browser; you get a clean, printable request you can turn into a priced change order. No signup, nothing stored.

Priority

Nothing is stored. Fill this in the browser, copy or print, and send it to your freelancer.

How to use it

  1. Send your client this page (or fill it together on a call) when they ask for something outside the original deal.
  2. They enter the project, a short title, what they want changed, why it matters, priority and a desired date.
  3. Optional: they add a rough hour guess and a note with links or constraints.
  4. Copy the finished request or print / save as PDF and send it back to you.
  5. You reply with either "that's in scope" or a priced change order they approve before work starts.

Unsure whether the ask is even in scope? Run it through the free scope checker, or reply with one of the out-of-scope reply scripts while you price it.

Turn every approved request into a change order

ScopeLine stores your original deal, logs each client request against it and turns extras into change orders the client approves through a link — no login for them. Free for 3 active projects, no card.

FAQ

What is a freelance change request form?
A change request form is the short intake a client fills when they want something new or different mid-project. It captures what they want, why, priority and timing so the freelancer can decide if it is in scope or needs a priced change order — before unpaid work starts.
How is a change request different from a change order?
A change request is the client's ask. A change order is the freelancer's priced response: scope delta, fee, timeline and payment terms the client approves in writing. Use this form for intake, then a change order template (or ScopeLine) once you know the price.
Should clients fill this form themselves?
Yes when you can. Having the client write the request reduces "I thought you meant…" debates later. Freelancers can also fill it on a call and send the copy back for confirmation. Either way, get it in writing before estimating.
Is any of this data stored?
No. The form runs entirely in your browser. Nothing is saved on ScopeLine's servers. Copy or print the document if you want to keep a record.
Is this form a legal document?
It is plain-English intake wording, not a contract and not legal advice. It does not approve work or set a price. Pair it with a signed change order or contract amendment before starting the extra work, and check local law if you need binding terms.
